Form OMB Expiration Tracker

Every VA form that the public fills out needs an active OMB number, an expiration date, and a revision date. The revision date is the last time the form was updated — usually every 3 years, but sometimes sooner.

This dashboard shows only the forms that need attention right now, across all the places these values can get out of sync:

Expired or expiring PDFs
Introduction pages showing the wrong expiration date or OMB number
Stale or duplicate PDF templates in vets-api

Forms that are current are counted but not listed. The dashboard refreshes once a month when the form-watch workflow runs: Slack gets the change alerts, while this page shows the current problems.

Data still needs human review. For example, a flagged PDF might be mid-way through an extension or other changes, or be leftover code from an old duplicate PDF in vets-api that hasn't been cleaned up yet.

This table shows forms whose OMB expiration date (per the PRA inventory) is expired, lapsed, or expiring within 90 days. When a renewal is approved, the introduction page (on vets-website) and the PDF template (in vets-api) need to be updated with the new expiration date or OMB number. 59 other watched forms have current expiration dates and are not listed.

PDF revisions or retired forms

This table shows when a form's PDF fields (per the VA Forms API) have changed, or the form has been retired (no longer available). New revisions to the PDF are alerted in Slack with a field difference against the current committed template. If field revisions have occurred, the online version of the form may need updating as well as the OMB data. 65 other watched forms' PDFs are available and current.
